We are seeking a skilled and compassionate Community Staff Nurse to join our team at Hounslow and Richmond Community Healthcare NHS Trust. As a key member of our Community Nursing Service, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes.
About the Role
As a Community Staff Nurse, you will be responsible for providing skilled nursing care to housebound patients with complex needs, enabling them to remain in their own homes. You will work collaboratively with our integrated rapid response teams, community matrons, and social service colleagues to ensure seamless care to our patient group.
Key Responsibilities
- Deliver high-quality nursing care to housebound patients with complex needs
- Work collaboratively with specialist nurses, allied health, and social care professionals to meet the needs of patients without unnecessary attendance in A&E or hospital admission
- Conduct first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care
- Be flexible to support the corporate approach to patient care as and when required
