Community Nurse

We are seeking a highly skilled and dedicated Community Nurse to join our team at Hounslow and Richmond Community NHS Healthcare Trust. As a Community Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals in their own homes.
Key Responsibilities- Deliver skilled nursing care to housebound patients with complex needs, enabling them to remain in their own homes.
- Work collaboratively with specialist nurses, allied health professionals, and social care colleagues to meet the needs of patients without unnecessary attendance in A&E or hospital admission.
- Conduct first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care.
- Be flexible to support the corporate approach to patient care as and when required.

Requirements- Level 1 Registered Nurse with valid registration with the NMC.
- Experience or recognized qualification in areas such as Health Promotion, Diabetes, Care of the older person, Continence, Palliative care, Medical/Surgical Nursing.
- Recent experience of Nursing at Level 5 or evidence of a back to nursing programme.
- Experience of working in the community setting.
We offer a competitive salary of £34,521 to £41,956 a year pro rata inclusive of HCAS (Outer). We also offer a range of benefits, including access to a motor vehicle for use at work and a UK driving license.
