Mental Health Community Nurse

We are seeking a skilled and compassionate Community Psychiatric Nurse to join our Adult Community Mental Health Team. As a key member of our team, you will play a vital role in delivering high-quality mental health services to our patients.
Key Responsibilities:
- Participate in multi-disciplinary assessments of patients' health conditions.
- Formulate and implement care plans.
- Provide treatment, as necessary.
- Use evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions.
- Collaborate with other healthcare professionals to ensure optimal patient care.
- Maintain accurate and up-to-date records in line with professional standards.
- Stay current with the latest mental health advancements and best practices.
Requirements and Qualifications:
- Current and valid registration with the NMC.
- Proven experience within a community recovery role.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing high-quality patient care.
