Pick for Britain is currently seeking an experienced adult care coordinator to join our team.

Job Summary: The successful candidate will be responsible for undertaking assessments of individual and carers' needs and risks to encourage independence, providing support and preventing harm.

Key Responsibilities:

  • To work in partnership with service users and carers using a person-centred approach, with the aim of maximising independence, wellbeing and choice.
  • To monitor implementation of packages of care and support and adjust them as appropriate.
  • To ensure that individual care packages, including funding arrangements for any services, are thoroughly reviewed in accordance with statutory requirements and Council policy.

Requirements:

  1. Child/ Adult DBS on Update Service.
  2. UK Passport/ Right to Work.
  3. Social Work England/ HCPC Check.
  4. The ability to work flexibly and manage competing priorities in a fast-paced environment.
