Key Responsibilities- Deliver individualized care and support plans to service users, focusing on their unique needs and goals.
- Work collaboratively with Care Coordinators, Key Workers, and other healthcare professionals to ensure seamless care delivery.
- Provide emotional support, practical assistance, and advocacy to service users, promoting their independence and dignity.
- Assist with daily living activities, including personal care, and facilitate community and hospital appointments as needed.
- Monitor service user progress, identify early signs of relapse, and alert relevant staff to ensure timely interventions.
- Maintain accurate records, adhere to health and safety protocols, and contribute to a safe and respectful work environment.
- NVQ Level III or Certificate in Community Mental Health, or equivalent experience.
- Experience working with individuals with mental health needs, with a commitment to ongoing personal and professional development.
- Basic IT knowledge and ability to maintain accurate records.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team.
- Flexibility to meet service needs, with a commitment to providing person-centered care.
