About the Role

We are seeking a highly skilled and experienced Clinical Assessor to join our Digital Care Hub team at Airedale NHS Foundation Trust. As a key member of our team, you will play a vital role in supporting the delivery of our new Harrogate Goldline Service, which aims to provide high-quality, patient-centered care to those in need.

Key Responsibilities
  • Provide remote clinical assessments and develop personalized care plans for patients accessing our services.
  • Support and care for palliative and end-of-life patients and their families, utilizing your expertise in clinical assessment and observational skills.
  • Collaborate with acute, primary care, and community teams to manage sick patients and ensure timely responses to remote monitoring alerts.
  • Participate in in-service training, preceptorship, and supervision of junior staff, promoting a culture of self-development and continuous learning.
  • Develop strong working relationships with nursing colleagues and the multi-disciplinary team, promoting quality care and non-discriminatory practice.
Requirements
  • Significant experience in palliative or acute care, with a strong understanding of clinical governance and risk management.
  • Ability to work with and motivate nurses, therapy, and medical staff, promoting a culture of teamwork and collaboration.
  • Strong communication and interpersonal skills, with the ability to develop good working relationships with colleagues and patients.
  • Knowledge of National Health Service agenda and relevant clinical professional agenda, with a willingness to learn new technologies related to digital and remote healthcare.
