About the Role

We are seeking a highly skilled and experienced Senior Clinical Assessor to join our Digital Care Hub team. As a key member of our team, you will play a vital role in supporting the delivery of digital services across Airedale and Craven.

Key Responsibilities
  • Provide high-quality, evidence-based remote clinical assessments to establish plans of care for patients accessing services in the Digital Care Hub.
  • Support and provide care for palliative/end of life patients and their family/carers who access the Goldline service via telephone.
  • Participate in the provision of early intervention with appropriate assessment, observation, and treatment for patients, utilizing clinical assessment and observational skills.
  • Ensure timely responses to all remote monitoring alerts.
  • Work with individual care plans to provide personalized care for all patients, referring to other services as required.
  • Collaborate with acute, primary care, and community teams in the management of sick patients.
  • Participate in in-service training, preceptorship, and supervision of junior staff, post-basic course nurses, and students (or AHPs as appropriate), with a commitment to self-development.
  • Develop good trust-wide working relationships and communicate effectively with nursing colleagues and members of the multi-disciplinary team.
  • Promote quality care and non-discriminatory practice as outlined in trust policies and procedures, acknowledging a patient's personal beliefs and identity.
About Us

We are a national centre for telemedicine and introduced telehealth to the UK offender healthcare sector. We provide services to patients in their care homes and are supported by 400 dedicated volunteers. We have strong links with Bradford University, Craven College, and Leeds City College to ensure we inspire the workforce of the future.

Person Specification

We are looking for a candidate with the following qualifications and experience:

  • RN/Project 2000 Adult Branch, mental health/LD RGN/BSc (or equivalent) AHP Qualification e.g. OT/PT/SALT
  • Non-Medical Prescribing/Advanced Clinical Assessment (or willing to work towards)
  • Current NMC/HCPC registration
  • Diploma in a relevant healthcare topic/working towards
  • ENB 998/C & G 730/SLiP Educational qual.
  • Over 2 years of Band 5 clinical practice

  • Research, teaching, and leadership skills
  • Willing to learn new technologies related to digital and remote healthcare
  • Assessment skills of the acutely ill patient
