About the Role

We are seeking an experienced General Practice Pharmacist to join our team as a Clinical Pharmacy Lead. This is an exciting opportunity to work in a dynamic and innovative environment, integrating clinical pharmacy skills into the general practice team.

The successful candidate will be responsible for providing clinical pharmacy leadership, taking responsibility for Structured Medication Reviews, chronic disease management, medicines optimisation, quality improvement, and other enhanced services. They will work closely with the GP partners to develop and implement clinical pharmacy services that improve patient care and outcomes.

The ideal candidate will have a strong background in clinical pharmacy, with experience in teaching, training, and mentoring. They will be able to develop collaborative working relationships with healthcare professionals and lead multidisciplinary clinical teams. A Certificate in Non-medical Prescribing and experience of using prescribing qualifications in General Practice are essential for this role.

Key Responsibilities
  • Provide clinical pharmacy leadership and support to the general practice team
  • Develop and implement clinical pharmacy services to improve patient care and outcomes
  • Work closely with GP partners to develop and implement clinical pharmacy services
  • Lead and coordinate the work of less experienced pharmacy professionals
  • Provide expertise in clinical medicines advice and address public and social care needs of patients
Requirements
  • Minimum of 5 years post-registration experience with at least 2 years in General Practice
  • Certificate in Non-medical Prescribing
  • Experience of using prescribing qualifications in General Practice
  • Evidence of further education in clinical pharmacy
