We are seeking a compassionate and experienced Social Worker to join our dedicated team in South Tyneside, UK. As a Social Worker - Adults with Learning Disabilities Specialist, you will work closely with individuals and their families to assess needs, develop care plans, and ensure they receive the necessary support to enhance their quality of life.
Key Responsibilities- Conduct comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
- Develop and implement personalized care plans that promote independence, well-being, and social inclusion.
- Work collaboratively with multi-disciplinary teams, external agencies, and community organizations to coordinate care and services.
- Ensure all interventions comply with relevant legislation, policies, and procedures related to adult social care.
- Build strong, trusting relationships with individuals, their families, and carers to support their long-term needs.
- Qualified Social Worker registered with Social Work England.
- Proven experience in a social work role, particularly with adults with learning disabilities.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with individuals, families, and professionals.
- Knowledge of relevant legislation, policies, and procedures related to adult social care and learning disabilities.
