Job Summary:
We are seeking an experienced and compassionate Social Worker to join our team. As a Social Worker - Adults with Learning Disabilities, you will be responsible for providing high-quality care and support to individuals with learning disabilities.
Key Responsibilities:
- Assess and develop care plans to meet the needs of individuals with learning disabilities
- Provide emotional support and guidance to individuals and their families
- Collaborate with other healthcare professionals to ensure comprehensive care
- Conduct regular reviews and assessments to ensure the effectiveness of care plans
Requirements:
To be successful in this role, you will need:
- Experience working with adults with learning disabilities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of relevant legislation and policies
