About the Role

We are seeking a compassionate and experienced Social Worker to join our dedicated team in South Tyneside, UK. Key Responsibilities
  • Conduct comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
  • Develop and implement personalized care plans that promote independence, well-being, and social inclusion.
  • Work collaboratively with multi-disciplinary teams, external agencies, and community organizations to coordinate care and services.
  • Ensure all interventions comply with relevant legislation, policies, and procedures related to adult social care.
  • Build strong, trusting relationships with individuals, their families, and carers to support their long-term needs.
Requirements
  • Qualified Social Worker registered with Social Work England.
  • Proven experience in a social work role, particularly with adults with learning disabilities.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with individuals, families, and professionals.
  • Knowledge of relevant legislation, policies, and procedures related to adult social care and learning disabilities.
