Role Summary:
We are seeking an experienced Social Worker to join our team in Stoke-on-Trent. As a Social Worker, you will play a vital role in supporting adults with diverse needs, working collaboratively with colleagues, health professionals, and partner agencies.
Key Responsibilities:
- Manage a caseload of adult referrals, assessing needs and developing support plans
- Work closely with adults, carers, and partner agencies to ensure person-centred support
- Collaborate with multidisciplinary teams to deliver holistic support services
- Participate in regular case reviews, supervision, and peer support to ensure high-quality practice
Salary and Benefits:
£38,000 - £48,000 per annum (dependent on experience) plus a comprehensive benefits package including regular training, a supportive team environment, and opportunities for career progression.
