ZipRecruiter is seeking a skilled Nurse Practitioner to perform comprehensive health risk assessments for Medicare Advantage members in their homes. As a key member of our team, you will evaluate patients' physical, mental, and social needs without the responsibility for ongoing care.

The ideal candidate will have a strong background in patient care, with a minimum of 3 years of experience preferably in primary care, adult or geriatric settings. You should be proficient in electronic medical records (EMR) systems and possess excellent verbal and written communication skills.

This per diem position offers flexible scheduling, allowing you to customize your schedule and earn supplemental income. You will be reimbursed for mileage at $0.67 per mile after the first 20 miles and receive paid training. Our company covers malpractice insurance, and a collaborating physician is assigned to ensure seamless care.

**Responsibilities:**

  • Conduct comprehensive in-home health risk assessments on Medicare Advantage members.
  • Identify and document all current and chronic health conditions, assessing physical, mental, and social needs.
  • Provide patient education and counseling on identified conditions and screening procedures.

**Requirements:**

  • Active and unrestricted Nurse Practitioner license.
  • Minimum commitment of 18 hours per week, including one full 8-hour day.
  • Ability to work independently and autonomously.
  • Strong empathy, compassion, and interpersonal skills.

**Estimated Salary:** $50-$75 per hour based on location and experience. Competitive pay is based on per-patient assessment.
