At Avon & Wiltshire Mental Health Partnership NHS Trust, we are seeking a dedicated and skilled Care Home Education Nurse Specialist to join our team.
Job Description:This is an exciting opportunity for a motivated and experienced nurse to work as an independent practitioner delivering high-quality interventions to people residing in care homes in the Wiltshire area.
Responsibilities:- Deliver rapid and prioritized specialist mental health assessments with recommendations for care and treatment within multiple care pathways.
- Determine the nature and severity of mental health needs with consequent signposting and pathway facilitation.
- Ongoing education and training for the non-specialist workforce.
The successful candidate will be expected to contribute evidence-based expertise to multi-disciplinary team care planning processes, hold a caseload of service users requiring monitoring and review of medication regimes, and work in partnership with service users and carers providing them with relevant information and signposting them to relevant agencies.
We offer a competitive salary of £38,890 - £44,997 per annum, depending on experience, and a range of benefits including opportunities for professional development and a supportive working environment.
