Job Summary:
We are looking for a highly experienced Registered Nurse with expertise in home health care to work as a Travel RN – Home Health Mobile I. The ideal candidate will have at least 2-3 years of nursing experience, a valid MD License, and knowledge of home health and HCHB practices.
Responsibilities:
- Deliver high-quality patient care in the comfort of their own homes
- Conduct thorough assessments, develop personalized care plans, and monitor patient progress
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Provide education and support to patients and their families on disease management and self-care techniques
