We are seeking a highly skilled and compassionate Personal Care Coordinator to join our team in the ST7 Stoke on Trent area of Staffordshire.

This role involves working with individuals who require ongoing support due to complex medical conditions, such as Multiple Sclerosis, Osteomyelitis, Ileostomy, Supra pubic catheter, Emphysema, and cough assist.

As a Personal Care Coordinator, you will be responsible for creating and implementing personalized care plans that meet the unique needs of each individual.

You will work closely with our multidisciplinary team to ensure seamless delivery of care and support.

Salary: £15.01-£18.27 per hour (Weekly pay)

Responsibilities:

  • Create and implement personalized care plans that meet the unique needs of each individual
  • Work closely with our multidisciplinary team to ensure seamless delivery of care and support
  • Provide high-quality care and support to individuals with complex needs
  • Assist with daily living tasks, including medication management and administration

Requirements:

  • Excellent communication skills
  • Driver's licence is essential
  • Good professional and personal references
