Care Assessment and Management Professional

We are seeking an experienced Care Assessment and Management Professional to join our team at the Black Country Integrated Care Board (ICB). This is a part-time position, requiring 18.75 hours of work per week.
As a Care Assessment and Management Professional, you will play a crucial role in improving the lives of individuals through high-quality care assessments and management. You will be responsible for conducting thorough assessments for NHS Continuing Healthcare for Adults, coordinating and monitoring complex care packages, and strengthening links between primary care, social services, and other stakeholders.
The ideal candidate will have excellent time management skills, good working knowledge of adult safeguarding, awareness of Data Protection and confidentiality issues, and working knowledge of Microsoft Office.
