Vacancy Overview

We are seeking a skilled Social Worker to join our team at West Berkshire Council. As a Social Worker, you will play a vital role in supporting vulnerable individuals and promoting their well-being.

Key Responsibilities:

  • Supporting service users in their safe discharge from hospital through a joint Health and Social Care approach.
  • Assessing and reviewing eligibility for long-term support in accordance with the Care Act 2014.
  • Working with vulnerable individuals who require support in accordance with the Mental Capacity Act 2005.
  • Promoting the rights and safeguarding of individuals at risk of abuse.
