As an Adult Social Work Professional within their Reablement & Sensory Needs Team, you will contribute to the delivery of high-quality care services. This full-time position is based in West Berkshire with a competitive salary of up to £41,418 per annum.

Key Responsibilities:
  • Participate in the link worker role by conducting bedside assessments and arranging packages of care for safe discharges.
  • Be part of the duty rota systems covering several days a week in the hospital setting and responding to referrals as and when received.

Key to your success is ensuring the smooth discharge of patients from acute hospitals. This involves monitoring delays and feeding back to the office/providers, chasing daily to ensure timely resolutions.

To continue delivering exceptional care, you will:

  • Develop and maximise the benefits of links with services provided in the NHS, voluntary, and private sectors to ensure customers receive the best possible support.
  • Ensure the continued professional development record is maintained.
  • Develop professional networks to achieve your personal work objectives.

In this role, you will also:

  • Ensure the team uses resources effectively by making appropriate judgements regarding the provision of information, advice, dealing with simple short-term interventions and referring to other teams.
  • Support in managing the performance of staff on a day-to-day basis through implementation of Council policies and procedures – especially those relating to Safeguarding and Mental Capacity Act.

As a professional Social Worker, you will require a Social Work qualification or equivalent.
