About the Role

We are seeking an experienced Palliative Care Nurse to join our team at Thames Hospice, providing high-quality care to patients requiring end of life care in their own homes.

Key Responsibilities
  • Lead a team of nurses for a shift and provide individual care to patients based on their needs.
  • Work collaboratively with the multidisciplinary team to ensure a coordinated and defined management plan for each patient.
  • Communicate effectively with patients, families, and professional colleagues to ensure a safe and needs-led care pathway.
  • Participate in training and development opportunities to enhance clinical skills and knowledge.
Requirements
  • Registered Nurse with experience in palliative care.
  • Knowledge and understanding of current developments in palliative care.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain accurate healthcare records and use multiprofessional records to document patient care.
