Care Coordinator Opportunity

Voyage Care is seeking a skilled Care Coordinator to join our team in Gloucester. As a Care Coordinator, you will play a vital role in ensuring the people we support are kept safe by effectively deploying our care delivery colleagues and managing administration in line with relevant regulations and legislation.

Key Responsibilities:

  • Manage and coordinate the care delivery team to ensure seamless care delivery.
  • Ensure relevant documentation is in place for new individuals we support, discharges, and transfers.
  • Work closely with the care delivery team to ensure effective communication and coordination.
