We are seeking a Primary Care Practitioner to join our team at Lancaster Medical Practice. The successful candidate will be responsible for working with patients to identify and address their mental health needs.

About the Role

This is a challenging and rewarding role that requires excellent communication and interpersonal skills. The Primary Care Practitioner will work closely with GPs, ACPs, and other healthcare professionals to deliver high-quality patient care.

Key Responsibilities
  • Maintaining patient records and producing care plans and risk assessments, including assessment of the physical health of patients.
  • Working with patients' families and carers, helping to educate them and the patient about their mental health problems.

The estimated salary for this role is £50,000 - £70,000 per annum, depending on experience.
