Job Description for Family Health Practitioner

As a Family Health Practitioner at Hightown Surgery, you will be responsible for providing high-quality healthcare services to our patients, particularly children and families. Your duties will include:

  • Conducting consultations with patients, either in-person or over the phone
  • Visiting patients at their homes, as needed
  • Managing patient records and prescribing medications in accordance with our prescribing formulary
  • Working collaboratively with other healthcare professionals to ensure comprehensive care for our patients
  • Participating in the Enhanced Access Rota and taking part in home visiting sessions

You will be required to have excellent communication and interpersonal skills, as well as the ability to work autonomously and make professional decisions in relation to patient care.
