About the Role

We are seeking an experienced Mental Health Specialist to join our team in Colwyn Bay. As a Community Care Coordinator, you will be responsible for coordinating care services for individuals with severe and enduring mental health problems.

The ideal candidate will have excellent communication skills, be able to work independently, and possess a strong understanding of mental health issues. You will also have experience in developing and implementing care plans, as well as assessing and managing risk.

Key Responsibilities:

  • Care coordination and case management for clients with complex needs
  • Ensuring adherence to care and treatment plans
  • Formulating and developing outcome-focused care plans
  • Assessing and managing clinical risk
  • Supporting clinical duties, including depot and clozapine clinics

What We Offer:

  • A competitive salary of £35,000 - £45,000 per annum

Requirements:

  • Registered Mental Health Nurse qualification
  • Experience in mental health care coordination or a related field
