Job SummaryAs a Care Coordinator, you will play a crucial role in ensuring that clients receive the appropriate support and services to enhance their well-being. You will be responsible for overseeing and coordinating care services for individuals in need of assistance.
Key Responsibilities- Assessing clients' care needs and developing personalised care plans
- Coordinating with healthcare professionals, social workers, and family members to provide comprehensive care
- Monitoring and evaluating the effectiveness of care plans
- Scheduling appointments and arranging transportation for clients
- Maintaining accurate records of client interactions and progress
- Providing emotional support and guidance to clients and their families
You'll gain an in-depth understanding of the roles and responsibilities involved in working within the NHS, as well as knowledge of the systems used to effectively complete your role to the highest standard. You'll also develop a sense of teamwork, gained through working alongside and supporting colleagues from all levels within the organisation.
RequirementsEducation to NVQ 3 Level or equivalent in a relevant subject, such as Administration, IT, or Health/Social Care. Significant equivalent previous proven experience is also acceptable. You'll need to be a clear communicator with excellent verbal, writing, data entry, and presentation skills. Intermediate knowledge of IT systems, databases, and software programs such as Outlook, Word, Excel, and PowerPoint is also required.
