We are seeking a highly skilled and experienced manager to join our team as our North Yorkshire Hospital Discharge Coordinator. As our North Yorkshire Home from Hospital Manager, you will be responsible for managing our Home from Hospital Coordinators, ensuring that individuals receive the support they need to return home safely and confidently after a hospital stay.

The successful candidate will have at least 3 years of management experience, with a proven track record of successfully managing high-performing teams. You will be able to influence referral partners, promote and develop the service, and work effectively in multi-disciplinary teams within hospital environments.

In this role, you will be responsible for developing and implementing strategies to improve the effectiveness of our Home from Hospital service, working closely with hospitals, GPs, and other stakeholders to achieve safe and timely hospital discharges.

The salary for this role is between £32,076 and £34,684, depending on experience. If you are a motivated and experienced manager who is passionate about improving healthcare services, we encourage you to apply for this exciting opportunity.
