We have an exciting opportunity for a skilled manager to join our team as our North Yorkshire Home from Hospital Service Manager. This role involves managing our Home from Hospital Coordinators working across North Yorkshire, ensuring that individuals receive the support they need to rebuild confidence and independence after a hospital stay.

The ideal candidate will have at least 3 years of management experience, with a proven track record of successfully managing geographically dispersed teams. You will be able to influence referral partners, promote and develop the service, and work effectively in multi-disciplinary teams within hospital environments.

In this role, you will be responsible for developing and implementing strategies to improve the effectiveness of our Home from Hospital service, working closely with hospitals, GPs, and other stakeholders to achieve safe and timely hospital discharges.
