Public Health Nursing Professional

About the Role
The Community Health Worker will work as part of the public health nursing team to provide preventative and supportive healthcare to children, young people, and their families. The post holder will work under the supervision of a Specialist Public Health Nurse or Team Leader with a defined population.
Main Responsibilities
- Support the delivery of universal services and targeted interventions to address health inequalities.
- Work in partnership to provide appropriate advice and support to families in accordance with evidence-based practice under the supervision of a Specialist Community Public Health Nurse (SCPHN).
- Signpost families to supportive services, including Children's Centres and services provided by the local authority or the voluntary sector.
- Maintain clear and accurate records as per standards and guidelines, ensuring clear analysis and outcomes for the child.
Requirements
- QCF or equivalent qualification (e.g., NNEB, CACHE, BTECH Diploma, NVQ 3 in Childcare)
- Evidence of continuing professional development
- Ability to work as a reflective practitioner
- Group and family work experience
- Child Protection awareness
