We are seeking a highly skilled and compassionate Registered Mental Nurse (RMN) to join our Community Mental Health Team for Older People in Weymouth. As a key member of our team, you will work closely with individuals suffering from severe mental illness, providing person-centered care and support to aid their recovery and well-being.
Key Responsibilities- Assess, plan, implement, and evaluate programs of care for patients with complex mental health needs.
- Conduct risk assessments and develop effective management plans to ensure patient safety.
- Work collaboratively with multidisciplinary teams, including psychiatrists, psychologists, and support workers, to provide comprehensive care.
- Develop and maintain strong relationships with patients, carers, and other professionals to ensure seamless care coordination.
- Participate in duty rota and work collaboratively with the team to ensure effective service delivery.
