About the Role

We are seeking a highly skilled Nurse Care Coordinator to join our team in Alabama. As a key member of our care coordination team, you will be responsible for providing in-home and telephonic care coordination services to beneficiaries across the United States.

Key Responsibilities
  • Provide in-home and telephonic care coordination services to beneficiaries, including assessing their needs, developing personalized care plans, and coordinating with healthcare providers to ensure optimal health outcomes.
  • Foster an environment that promotes the Acentra Health 'caring' philosophy, incorporating empathy and compassion into all aspects of care coordination.
  • Collaborate with interdisciplinary teams to develop and implement effective care plans that address the unique needs of each beneficiary.
Requirements
  • Bachelor's degree in Nursing (BSN)
  • Public Health Nursing experience and geriatric nursing care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including beneficiaries, caregivers, and healthcare providers.
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
  • Proficiency in Microsoft Office, electronic medical record systems, and multiple computer applications.
