Senior Nurse - Hospice Community Services

This is an exciting opportunity to join our Hospice Community Services team, a unique and award-winning flagship service that offers innovative models of care. We work collaboratively with other providers to deliver high-quality care that meets the needs of our patients and their families.

We assess needs and plan, implement, and deliver tailored packages of care that truly make a difference in the lives of our patients. Our team is dedicated to providing seamless support in the community, and we are looking for a skilled and compassionate Senior Nurse to join us.

Main Responsibilities
  • To use telephone triage to assess patient and family need, providing advice and support.
  • To offer face-to-face assessments to provide the best possible care for patients at the end of life, meeting their choices and supporting their families and carers.
  • To assess and review care support in any setting, including home, hospital, care home, and Hospice.
  • To be aware of responsibilities and processes in safeguarding vulnerable adults and children.
  • To promote personal and professional development of all nursing staff.
Requirements
  • Registered Nurse Qualification and NMC pin number.
  • A commitment to work in Palliative Care Nursing.
  • Clinical Assessment and Diagnosis Skills.
  • Community nursing experience.
  • Excellent written and verbal communication skills.
  • Flexibility and willingness to work on rotating shift patterns.
  • Advanced Non-Medical Prescribing desirable but not essential.
