We are seeking a highly skilled and experienced Care Coordinator to join our team at Active Care Group. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.
Key Responsibilities:
- Coordinate and schedule care packages to ensure continuity of care for clients in their own homes.
- Provide premium customer service to clients and their families, responding to their needs and concerns in a timely and professional manner.
- Work closely with our care team to ensure that all care services are delivered in accordance with our company's policies and procedures.
- Monitor and report on the quality of care services, identifying areas for improvement and implementing changes as necessary.
- Collaborate with our clinical and business leadership teams to develop and implement new care services and programs.
- Ensure compliance with all relevant laws, regulations, and industry standards.
Requirements:
- Experience as a Care Coordinator or in a similar role.
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and care staff.
- Ability to work independently and as part of a team, with a strong focus on customer service and quality care.
- Knowledge of care policies and procedures, with the ability to apply this knowledge in a practical setting.
