About the Role

We are seeking an enthusiastic and dedicated Adult Mental Health Practitioner to join our Community Mental Health Team in Bridgwater. As a key member of our team, you will play a vital role in providing high-quality care and support to adults with mental health needs.

Main Responsibilities
  • Manage a caseload of patients and provide individualized care and support to promote independence, social inclusion, and recovery.
  • Work collaboratively with the Operational Team Manager, Clinical Leads/Practitioners, and other community teams to ensure seamless care coordination.
  • Conduct assessments, develop care plans, and evaluate progress in conjunction with clients and carers.
  • Provide supervision to junior staff and coordinate the care of clients, delegating tasks as necessary.
Requirements
  • RMN, Social Work, or Occupational Therapy Qualification
  • Current registration with relevant professional body
  • Evidence of post-qualifying professional development in an acute setting
  • Significant post-registration experience in multi-disciplinary community mental health services
  • Good knowledge of resources and working practices across health and social care
