Join Our Community Mental Health Team

We are seeking an enthusiastic and dedicated Adult Mental Health Practitioner to join our Community Mental Health Team in Bridgwater. As a key member of our team, you will play a vital role in providing high-quality mental health services to adults in need.

About the Role

This is an exciting opportunity to work in a supportive and compassionate multi-disciplinary service where every person is an integral part of the team. You will work closely with our Operational Team Manager, Clinical Leads/Practitioners, and community teams to provide comprehensive mental health support to our patients.

Key Responsibilities
  • Manage a rolling caseload of patients, monitoring their mental state, promoting independence, and reducing hospital admissions.
  • Work collaboratively with the Operational Team Manager, Clinical Leads/Practitioners, and community teams to provide comprehensive mental health support.
  • Develop and implement care plans in conjunction with clients and carers.
  • Provide supervision to junior staff and coordinate the care of clients.
  • Participate in team days, nights out, and other activities to foster a positive team culture.

Requirements
  • RMN, Social Work, or Occupational Therapy Qualification.
  • Evidence of post-qualifying professional development in an acute setting.
  • Significant post-registration experience in multi-disciplinary community mental health services.
  • Good knowledge of resources and working practices across health and social care.
  • Experience of managing risk of clients in crisis.
  • Ability to inspire hope, support recovery, and make a difference.
