Adults with Learning Disabilities Outreach Manager

We are seeking an experienced and dedicated Manager to lead our Outreach Service for Adults with Learning Disabilities in South Camden. The successful candidate will have a passion for delivering high-quality services and a commitment to working collaboratively with service users to maximise independence, self-confidence, and integration in the community.
The role will involve coordinating and providing person-centred care and support to service users, enabling them to live independently in their own homes and receive support in all areas of day-to-day living. You will also oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to people with learning disabilities.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. Key Responsibilities:
- Lead and manage the Outreach Service team to deliver high-quality services to adults with learning disabilities.
- Develop and implement care plans to support service users in achieving their goals and maximising their independence.
- Collaborate with service users, families, and other stakeholders to ensure person-centred care and support.
- Oversee the development and implementation of new activity groups and initiatives to promote social interaction, learning, and wellbeing for service users.
- Manage and coordinate the service to ensure effective use of resources and efficient delivery of services.
Requirements:
- Experience of leading and managing a team supporting people with learning disabilities or another vulnerable client group.
- Knowledge and understanding of the needs and challenges faced by adults with learning disabilities.
- Ability to develop and implement effective care plans and support strategies.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with service users, families, and other stakeholders.
