Responsibilities of the Hospital Pharmacist Role:- Provide Patient Support: Offer personalized guidance to patients regarding medication usage, potential side effects, and drug therapy. Engage with other healthcare professionals and discuss treatment plans with relevant parties.
- Manage Prescriptions: Accurately prescribe and review medications to ensure their suitability and effectiveness. Provide recommendations for optimal medication forms and facilitate patient discharge by transferring relevant information to primary care and coordinating ongoing treatment.
- Ensure Medication Safety: Participate in ward rounds to gather patient drug histories, guarantee safe and effective medication use, and report any medication errors or near misses.
- Identify Medicine-Related Issues: Recognize and address medicine-related concerns to minimize hospital admissions and readmissions.
