Supporting Young Adults with Learning Disabilities

**Job Overview**
We are seeking an experienced Adult Social Worker to join our newly established Preparing for Adulthood Team in Greater Manchester. As a Transition Social Worker, you will play a critical role in supporting young adults with Learning Disabilities, Autism, or neurodiversity as they transition into adulthood.
Key Responsibilities:
- Deliver high-quality support to young adults and their families to ensure a smooth transition into adult services.
- Work collaboratively with the Preparing for Adulthood team to develop and implement effective transition plans.
- Provide timely interventions to address emerging needs and concerns.

Requirements
To be successful in this role, you will need:
- A recognised degree in Social Care and current registration with Social Work England.
- At least 2 years of experience in Learning Disabilities and/or Transition Social Work.
