About the Role
Hastings & Rother Healthcare is seeking an experienced Advanced Clinical Practitioner or Nurse Practitioner to join our team in Frailty and Elderly Care. As a key member of our multidisciplinary team, you will provide high-quality, patient-led care to our frail and older patients, working closely with GPs, Advanced Clinical Practitioners, Nurse Practitioners, and other healthcare professionals.
Key Responsibilities
• Provide face-to-face and telephone consultations with patients presenting with a wide range of minor illnesses, urgent, and same-day requests for treatment and chronic disease management.
• Work as part of a multi-disciplinary team to provide a high-quality, cost-effective service to our patients, focusing on frail and older people, and those with low-level mental health concerns.
• Conduct weekly ward rounds and visit patients in their homes to assess their medical condition and frailty review.
• Prioritize and triage patient needs, making any necessary referrals for investigations in the appropriate manner.
Person Specification
• Essential:
- Experience of working with frail and older patients
- Experience of providing specialized advice in relation to care
- Two+ years proven record of working as an ACP dealing with patients presenting with a wide range of medical issues
- Experience of dealing with patients with undifferentiated and undiagnosed conditions
Desirable:
- Previous experience in General Practice
Qualifications:
• Essential:
- Full and unrestricted NMC (UK) or HCPC registration
- RGN (degree or equivalent)
- Prescribing Qualification
- Qualification in Physical Assessment skills and Frailty
Knowledge and Skills:
• Essential:
- Evidence of continuing professional development
- Evidence of clinical knowledge in caring for patients in relevant specialism
- In-depth knowledge of recent NHS legislation, recommendations for specialist area
