Job Summary:
  • Deliver high-quality, person-centered care, focusing on emotional and practical support.
  • Contribute to assessments, identifying and meeting individual needs, ensuring a safe and comfortable living environment.
  • Engage with individuals, encouraging participation in activities and promoting independence.
Key Responsibilities:
  • Supporting individuals with day-to-day tasks, such as managing finances, preparing meals, and maintaining their home.
  • Monitoring and reporting changes in their health, providing updates to relevant authorities when necessary.
  • Administering medications and performing temperature checks as required.
  • Participating in regular reviews, assessing the effectiveness of care plans and making recommendations for improvement.
  • Collaborating with other care professionals to ensure seamless delivery of services.
