Key responsibilities include:
- Coordinating core care hours, shared hours, and 1-2-1 hours commissioned by local authorities.
- Ensuring 4 weeks of rotas are completed in advance and shared with local teams.
- Actioning requests for changes from Wellbeing Leaders and Locality Managers to meet individual needs.
- Allocating training and annual leave bookings on each local rota and arranging cover.
- Booking agency staff to cover shifts as required.
- Maintaining regular contact with our bank support work team and building strong relationships.
- Ensuring all rota changes are completed and shared with team members across the region.
- Updating and submitting weekly finance trackers for each service.
- Reporting on delivered hours to locality managers and Divisional Director.
- Completing weekly payroll returns for each location.
Requirements include:
- NVQ Level 3 in adult care or equivalent qualifications.
- Proficiency in MS Office and web-based services.
- Experience in care coordination, adult health, and social care, learning support, or public health/health improvement.
- Experience in multiprofessional team environments.
- Experience in personalised care and support planning.
- Data collection and using tools to measure service impact.
