An exciting opportunity has arisen for a full-time Primary Care Mental Health Practitioner to join our Solihull Neighbourhood Mental Health Team.
About the RoleAs a Primary Care Mental Health Practitioner, you will be responsible for providing mental health triage, assessment, and outcome-based interventions to meet service users' needs within the Primary Care setting.
You will work closely with GPs and other primary care professionals to achieve the best outcomes for individuals within the local community.
Main Responsibilities- Provide mental health triage, assessment, and outcome-based interventions to meet service users' needs.
- Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.
- Support and advise colleagues within the PCN and across local neighbourhood multi-agency teams.
- Ensure robust communication across mental health providers, VCSE organisations, and within primary care.
