Primary Care Mental Health Practitioner

About the Role
We are seeking an enthusiastic and dynamic Mental Health Practitioner to join our Primary Care team, working alongside GPs and other professionals to achieve the best outcomes for individuals in the community.
Main Responsibilities
- Provide mental health triage, assessment, and outcome-based interventions to meet service users' needs.
- Promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.
- Support and advise colleagues within the Primary Care Network and local multi-agency teams.
- Ensure robust communication across mental health providers, VCSE organisations, and within primary care.
