Job Summary

An exciting opportunity has arisen for a full-time Primary Care Mental Health Practitioner to join our team at Birmingham and Solihull Mental Health NHS Foundation Trust.

As part of our community mental health services, we are looking to increase our primary care teams with primary care practitioner roles. This is an excellent opportunity to be at the heart of national change and developments in the delivery of community mental health care.

We are seeking enthusiastic and dynamic individuals who are able to work within a multi-disciplinary team, working alongside GPs and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.

Main Responsibilities

You will be responsible for providing mental health triage, assessment, and outcome-based interventions to meet service users' needs.

You will promote psychosocial wellbeing by identifying and supporting pathways for service users and carers.

You will support and advise colleagues within the PCN and across local neighbourhood multi-agency teams.
