About the Role:
We are looking for a dedicated and compassionate Support Worker to join our team at Hamilton Court, Conisbrough.
As a Support Worker, you will play a vital role in supporting individuals with learning disabilities and associated physical disabilities to live independently and achieve their goals.
Your key responsibilities will include:
- Providing person-centred care and support to individuals in their daily living needs, including personal, nutritional, social, and emotional support.
- Facilitating stimulation and promoting independence in line with individual support plans.
- Supporting individuals to access new activities, opportunities, work, and leisure interests.
- Reviewing and updating person-centred support plans and risk assessments.
- Working collaboratively with the team to ensure a high-quality service is delivered.
Requirements:
To be successful in this role, you will need:
- A good understanding of enabling services, learning disabilities, safeguarding adults, health and safety, and data protection.
- Excellent communication and interpersonal skills.
- The ability to work positively within a team and contribute to a supportive and inclusive environment.
- Flexibility to work a rota pattern, including evenings, weekends, and bank holidays.
