About the Role

As an Integrated Care Lead at The Mid Yorkshire Teaching NHS Trust, you will play a crucial role in enhancing patient care and experience. This position requires a dynamic and motivated Registered Nurse to lead our Early Assessment Team (EAT) as a Band 6 Nurse Coordinator.

The successful applicant will have a strong understanding of early discharge and admission avoidance, considering interdisciplinary needs and implications for patients needing intermediate care. Experience in community nursing, primary care, and teaching is essential. Additionally, you should have excellent communication skills, presentation skills, and the ability to assess, plan, and implement nursing care packages.

This is an exciting opportunity for a self-motivated and enthusiastic individual to join our team. You will work collaboratively with Occupational Therapists, Physiotherapists, Social Workers, and Acute Hospital staff to prevent unnecessary hospital admissions, facilitate earlier discharges, and provide supportive care to patients.

Salary: £43,041 - £60,971 per annum (dependent on experience)
