At 5 Star Recruitment, we are seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in delivering high-quality social work services to older adults who are vulnerable or in need of support.
Key Responsibilities:- Provide direct social work services to vulnerable citizens, adhering to statutory, national, and regional policies and guidelines.
- Work in partnership with colleagues, private and voluntary sector organizations, health services, and emergency services to deliver an integrated response to meeting needs.
- Build effective relationships with citizens, families, and carers to identify and meet needs, ensuring the rights of individuals are safeguarded.
- Ensure timely completion of assessments and reviews, adhering to statutory timescales.
- Utilize Person-Centered Planning approaches to support citizen participation in assessment, care planning, and review processes.
- Respond to safeguarding concerns in accordance with the Wales Interim Policy and Procedure for POVA.
- Conduct POVA investigations and complete risk assessments.
- Contribute to the preparation and presentation of reports and plans at POVA Strategy meetings.
- Collaborate with Community Wellbeing services to develop knowledge and awareness of available resources, ensuring access to early and preventative interventions.
- Maintain professional accountability for resource allocation, adhering to service criteria, legislation, and financial regulations.
- Ensure an early and effective response to concerns or complaints from citizens or other agencies.
- Support citizens and their carers/families to actively participate in service development.
- Prepare for and contribute effectively to meetings, chairing meetings as required, and quality-assuring meeting records.
- Prepare written reports for various purposes, ensuring accuracy, coherence, completeness, and timeliness.
- Attend team meetings and contribute to service development.
- Maintain a specific caseload of varied complexity.
- Apply research, theory, evidence-based practice, and problem-solving methodologies to achieve good outcomes for citizens.
- Maintain good recording practices, ensuring records are accurate, coherent, up-to-date, and completed in a timely manner.
- Ensure casework is effectively planned and outcome-focused, with citizens, families, and carers fully represented.
- Complete time-specific tasks within timescales and respond to telephone, electronic, and written communications in a timely manner.
- Promote anti-discriminatory practices and act in a non-oppressive manner towards service users, carers, and colleagues.
- Provide a positive role model in relation to conduct within the workplace, including good workload and time management.
- Exercise responsibility under the Health and Safety at Work Act and respond promptly and robustly to these regulations.
- Assist with the duty system as needed and undertake backup duties on a regular basis.
- Transport/accompany citizens to appointments, as appropriate, both within and outside the Conwy County Borough area, following risk assessments.
- Maintain professional registration and comply with post-registration training and learning requirements.
- Prepare for and participate in professional supervision and performance development reviews to support personal development and adherence to professional standards.
- Attend training and seminars designed to develop personal and professional expertise, disseminating knowledge and learning to colleagues as appropriate.
- Establish good working relationships both internally and externally.
- Comply with the Authority's Policies and Procedures and report any inadequacies to Senior Officers.
- Participate actively in supporting the Authority's principles and practices of equality of opportunity, as laid down within the Equalities Policy.
- Apply Health & Safety practices within daily operations, sharing a common responsibility for Health & Safety across the department, directorate, and Authority.
- Provide certain information to the Authority for personnel, administrative, and payroll purposes.
- Undertake any other reasonable tasks, commensurate with grade, as determined by the Line Manager or Head of Service.
- Be aware of the Corporate Safeguarding Policy and report any concerns in the appropriate manner and timescales.
