About the Role

We are seeking a highly skilled and compassionate Adult Dementia CPN to join our team in Ealing Cognitive Impairment and Dementia Services. As a key member of our multidisciplinary team, you will provide specialist assessments, reviews, and nursing interventions to service users accessing our Cognitive Impairment & Dementia Service (CIDS).

Main Responsibilities
  • Conduct comprehensive assessments and develop care plans for individuals with dementia, ensuring their physical, social, and psychological needs are met.
  • Work collaboratively with service users, carers, and other healthcare professionals to deliver high-quality, person-centered care.
  • Administer and monitor medication, including depot injections, and provide information and advice to service users and their families.
  • Participate in local audit and research projects to broaden your research and development skills.
  • Contribute to the development and implementation of care pathways and protocols to ensure best practice in dementia care.
Requirements
  • Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
  • Evidence of continuous professional development, particularly in dementia care.
  • Experience of working with patients and their carers with complex health and social care difficulties, including dementia.
  • Ability to work within a multidisciplinary community team and contribute to the development of care plans.
  • Excellent communication and interpersonal skills, with the ability to work with people from diverse backgrounds.
