About the Role:

We are seeking a highly skilled Learning Transition Advisor to join our Supported Learning provision. The successful candidate will focus on ensuring smooth transitions for EHCP and High Needs Students, including attending Annual Reviews and school liaison.

The role will involve developing strong relationships with schools, feeder schools, special schools, and local authorities to plan provision and transition for learners with difficulties and/or disabilities.

You will work closely with the Supported Learning and SEND teams to ensure all relevant information is recorded and provided in a timely manner.

The ideal candidate will have experience working within education with High Needs and EHCP learners and be familiar with the Annual Review process of an Education Health and Care plan.

Key Responsibilities:

  • Develop links with schools, feeder schools, special schools, and local authorities to plan provision and transition for learners with difficulties and/or disabilities.
  • Work closely with the Supported Learning and SEND teams to ensure all relevant information is recorded and provided in a timely manner.
  • Attend annual reviews at schools and arrange observations of the learners who potentially will be attending the college.
