Neonatal Community Care Specialist

This role offers an exciting opportunity to work as a Neonatal Community Care Specialist within the Home Team at University Hospital Southampton NHS Foundation Trust. You will be responsible for assessing, planning, implementing, and evaluating individualised discharge plans for our neonatal and transitional care patients and their families.
The ideal candidate will have experience of working on a neonatal unit, understanding of personal accountability, and child and adult safeguarding awareness. They should also possess excellent organisational skills, ability to use own initiative, and effective communication skills.
As a member of the Home Team, you will support families with the transition from hospital to home, providing education and emotional support as necessary. You will continue to provide holistic family-centred care to patients in the home environment in partnership with their families, whilst supporting junior staff to do the same.
Salary: £32,000 - £43,041 per annum (dependent on experience)
