Job Description

We are seeking a highly skilled Social Worker in Community Care to join our Adult Social Care team. As a social worker, you will work closely with individuals with physical and sensory impairments, developing and implementing personalized care plans to support their independence.

The ideal candidate will possess a strong understanding of the CHC framework and S117 aftercare processes. They will also have excellent communication and interpersonal skills, enabling them to work effectively with colleagues, service users, and their families.

Key Responsibilities:

  • Develop and implement individualized care plans to support service users' independence.
  • Collaborate with health partners and other stakeholders to ensure seamless care transitions.
  • Provide emotional support and guidance to service users and their families.

Requirements:

  • Degree in Social Work
  • Registered with Social Work England (SWE)
  • Strong understanding of CHC framework and S117 aftercare processes
  • Excellent communication and interpersonal skills

Salary: £42,000 - £55,000 per annum
